About American Indian Health Service of Chicago

Provides direct health care, community health services, mental health care and supportive services without regard to race, color, sex, political and religious views. Special emphasis is placed on providing culturally sensitive services to American Indians. In addition, the center maintains a comprehensive health network to help clients in need of assistance beyond the scope of the health center. MEDICAL/DENTAL SERVICESMedical services include physical examinations, well child exams, pre-natal care, diabetic care, home visits, limited laboratory appointments. General dental care is provided for those ages 3 and up. Kid Care and Public Aid are accepted for dental services. There are set fees for dental services, with a discount for Native Americans. Doctor available Tuesday and Thursday 8:30AM-4:30PM and Friday 8:30AM- 2:00PM. Family Nurse Practitioner available 1st and 2nd Tuesdays of the month 5:30PM-8:00PM and 3rd and 4th Wednesdays and 2nd and 3rd Fridays of the month 2:00PM-5:00PM. Call for Dentist Clinic hours. Medical and Dental Clinics Located 4085 N Broadway, Chicago, IL 60613. DIABETES PROGRAMOperates the Healing Circle, A Community Diabetes Program for Urban American Indians. Program seeks to heighten the focus on prevention, screening and Diabetes awareness education to members of the Chicago urban American Indian community. Services include community education, home visits, health assessment, home nursing services, case management and referral. Patients can be followed for their diabetic care at the American Indian Health Service (AIHS) clinic. Diabetic patients will have regularly scheduled appointments depending on their medical condition in order to optimize the management of their diabetes and minimize the long-term complications of the disease. A sampling include foot screenings, medication and disease management, and laboratory workProgram operates Monday - Friday 8:30AM - 4:30PM. Diabetes Community Health program is located 4087 N Broadway, Chicago, IL 60613. MENTAL HEALTH CLINICMental Health department provides counseling and psychotherapy therapy to American Indians, Alaskan Natives, and their families and significant others. Individual and family therapy sessions are offered to encourage healthy relationships with self, others, family, and community. A samplings of presenting issues have included anxiety, depression, health related changes, esteem problems, career and life transitions, adult trauma, substance abuse, domestic violence and post traumatic stress disorder.Clinic Hours are by appointment only.
